WUTH works in close partnership with a number of specialist organisations to provide joined-up, seamless palliative care across the Wirral. Depending on your needs, you may receive support from one or more of the services below.

Wirral Hospice St John's

Located on the Clatterbridge Hospital site, offering specialist palliative and supportive care through Wellbeing, Outpatient, Inpatient and Hospice at Home services.

• Wellbeing services - group sessions and activity programmes
• Outpatient clinics - palliative medicine, counselling, complementary therapies
• Inpatient Unit - short-stay specialist care
• Hospice at Home - support at home in the final weeks of life
• Bereavement support for up to two years after death

Community & Night Nursing (WCHC)

Multiple community nursing teams across Wirral providing 24/7 nursing support at home and in care homes. The night service operates 5pm–8am, seven days a week.

WCHC Macmillan Specialist Palliative Care Team

Part of Wirral Community Health and Care NHS Foundation Trust, providing specialist palliative care for patients with advanced progressive disease - at home, in clinic, or virtually. Seven-day service.

• Clinical Nurse Specialists and Senior Nurse Practitioners
• Allied Health Professionals - social work, OT, physiotherapy, dietetics
• Face-to-face, telephone and virtual appointments

Claire House Children's Hospice

Providing palliative and end of life care for babies, children and young adults up to the age of 25 at home, in the hospice or in hospital. Works collaboratively with adult services for those aged 18–25.
